Replacement Certification Labels (vinyl label affixed to driver's door or door post) may be  available from Toyota providing the request meets one of the criteria listed in this TSB.

Certification Label Criteria

A replacement certification label may be requested if:

NOTE:
  • Processing of a replacement label will be delayed if the original (or copy/picture) certification label does NOT accompany the request. 
  • A replacement label MAY NOT be available if the vehicle is more than 5 years old.

Procurement Procedure

To request a replacement label, complete a copy of the form at the end of this bulletin. Your dealer parts account will be billed $40.00  for each replacement of a damaged or stolen label.

NOTE: All replacement labels for damaged and/or stolen vehicles are subject to approval by the Technical Compliance Department. If you have any specific questions, contact (310) 468-3390.
